How to put a header/logo image at the top of a wiki page is possible only if your wiki is on a Classic wiki functionality type . To change you wiki type:
  1. Go to your wiki
  2. Go Settings > General > Application Type
  3. Application Type = Classic Wiki Functionality - Scroll down & click SAVE
  4. On the Settings Page, go to Themes and Colors
  5. Select a theme that has an obvious logo/header area at top
  6. Once you do that, you will see a Logo option appear.

Create a survey using a Google form to determine how many people have faith in our governments Center of Disease Control ( CDC) properly protecting our country from an Ebola outbreak.
